Webinar 1: Health journey mapping – using Bronchiectasis to demonstrate the use of the clinical tool

This webinar will explain the benefits of using health journey mapping tools to enhance care for and with Aboriginal patients and clients in busy clinical settings using a clinical mapping tool.

The tools enable individuals and teams to identify and record priorities for care, map changes in real time, and evaluate effectiveness. They help improve communication and coordination of care, and can be used in urban, rural and remote locations, within and across services.

Participants will learn how the health journey mapping tools can be used alongside clinical guidelines to improve care experiences and outcomes for Aboriginal people with bronchiectasis.

Webinar 2: Health journey mapping - using Asthma to demonstrate the use of the detailed tool
Date and time: Tuesday 4 July 2023 | 2:00-3:30PM AEST

This webinar will explain the benefits of using health journey mapping tools to plan care for and with Aboriginal patients and clients using a detailed mapping tool.

The tools enable clinicians and teams to identify and record individual patient and client priorities for care, develop effective care plans, ensure continuity of care and follow up, and evaluate effectiveness. They can be used in urban, rural and remote locations, within and across services.

Participants will learn how the health journey mapping tools can be used to improve communication, care experiences and outcomes for Aboriginal children and adults with asthma, with a focus on discharge planning, client/patient/family involvement in developing care plans, and prevention of rehospitalisation.

  • Associate Professor Janet Kelly

Associate Professor Janet Kelly is a nurse researcher and course coordinator focused on improving health care and outcomes for and with Aboriginal people. She has co-designed flexible and adaptable health journey mapping tools that identify barriers and enablers, gaps and strategies in care. These tools are increasing being used for quality improvement, reflective practice, education and training in a range of health care areas including renal care, cardiac care, nursing, midwifery and medicine in urban, rural and remote areas.

  • Tahlee Blade Stevenson

Tahlee is a Registered Nurse and Researcher from South Australia. She has worked with the AKction team from the University of Adelaide since 2018, focusing on improving health journeys and outcomes for Indigenous Australians and imbedding cultural safety within clinical care. She has a public health background and is passionate about improving access to health services and achieving health equity for all. In addition to research, she works clinically as a Women’s and Reproductive Health nurse.
